Must be 21 due to company policy. Preferred degree in psychology, social work, sociology, education, rehab, nursing, or gerontology. Ability to identify, cultivate, and/or preserve potential partnerships with professional organizations. Execute specific outreach strategies to connect with community members, healthcare professionals, and families seeking services. Serve as one of the agency's marketing representatives for proactive outbound communication for programs and events. Develop multilingual content for program flyers, brochures, and presentations for diverse groups. Ability to deliver educational presentations (both in person and virtually) to large groups. Meets marketing metrics established by the Director of Communications in alignment with the company's strategic marketing plan. Aid patients in navigating the complex healthcare system, including understanding insurance and other healthcare benefits programs (Affordable Care Act, KidCare, Medicaid, etc.). Conducts awareness campaigns, including organizing events at various venues and providing group health education classes to support grant. Recommend strategies to improve the referral process. Ensure the completion of organizational benchmarks of engagement and productivity. Maintain information on all changes to the operational processes of the health center and educate referral sources on the services provided at the health center. In coordination with administration, the outreach coordinator forges new relationships and corresponding logistical workflows with referral sources to enhance continuity of care. Serves as a liaison for potential patients by facilitating future appointments. Knowledge of geographical areas and ethnic communities. Knowledge and understanding of customs, beliefs, and needs of consumer group(s) served. Knowledge of community resources and referral agencies. Solid analytical and problem-solving skills. Ability to analyze data and develop monthly reports for funders and the agency. Develop a referral process with our community partners to facilitate links to resources. Maintain records and reports of outreach efforts, community contacts, and partnership leads. Develops and maintains referral sources of community partners using tools like Salesforce or other similar platforms. Prepares weekly, monthly, and yearly reports and recaps to management and funders. Ability to attend and actively participate in staff meetings. Maintain a reliable record of punctuality and attendance. Be ready and available to work during regular business hours. Attend events after regular business hours to accommodate referral sources' schedules and community events. Must be willing to work flexible hours, including some evenings, weekends, and/or outdoor events. Ability to drive organizational vehicle to events when needed. Set up event/exhibit displays. Performs other duties as assigned. Workweek: Monday - Friday and weekends as needed.
