Queen's Health System Patient Care Coordinator, Cardiac Program (Full-Time, Day, 40) in Honolulu, Hawaii

RESPONSIBILITIES

I. JOB SUMMARY/RESPONSIBILITIES:

• Coordinates, manages, and supports the operations of the assigned cardiac program including the coordination of procedures.

• Responsible for the coordination of patient care throughout the continuum, in collaboration with healthcare providers and multidisciplinary care team, to provide quality, comprehensive services for assigned area(s).

• Serves as an advocate, educator and clinical resource for are(s) assigned.

• Serves as a liaison throughout the facility an in the community regarding services provided for this unique patient population.

• Coordinates all aspects of the perspective treatment plan regimen related to assigned area(s).

• Is the key driving force for steps and processes of the assigned program, ensuring a smooth clinic function every step of the way while navigating through multiple roles and responsibilities.

IV. TYPICAL PHYSICAL DEMANDS:

• Finger dexterity, seeing, hearing, speaking.

• Continuous: sitting, static gripping of an object for prolonged periods.

• Frequent: standing, walking, stooping/bending, and twisting body.

• Occasional: climbing stairs, walking on uneven ground, lifting and carrying usual weight of 1 pound up to 5 pounds, reaching above, at and below shoulder level, frequent gripping of an object.

III. TYPICAL WORKING CONDITIONS:

• Not substantially subjected to adverse environmental conditions.

VI. MINIMUM QUALIFICATIONS:

A. EDUCATION/CERTIFICATION AND LICENSURE:

• Current Hawaii State license as a Registered Nurse.

• Bachelor’s degree in nursing.

• Current BLS certification.

B. EXPERIENCE:

• Two (2) years nursing experience, preferably in a comparable organization in assigned specialty with demonstrated expert clinical leadership abilities.

• Demonstrated experience with electronic based medical records and word-processing applications.

• Experience to demonstrate:

o Ability to communicate clearly and concisely both verbally and in writing.

o Ability to manage rapidly changing clinical situations.

o Ability to function both independently and as a team member.

o Knowledge of current surgical and/or percutaneous management of vascular disease, as well as pharmacological treatment of adult cardiac surgery patients.

o Excellent customer service skills.

• Experience with spreadsheet application, clinical databases, and performance improvement activities preferred.
