ThePatient Access & Navigation Program Coordinator is essential in supporting various cancer programs, handling tasks such as managing multi-disciplinary conferences, coordinating patient tracking, collecting and organizing data and program registries, engaging in quality improvement initiatives, and promoting community outreach and education. Responsibilities include but are not limited to: · Maintain patient trackers, various excel spread sheets, program dashboards, and REDcap databases. Maintain patient databases and track recommended follow-ups · Prepare for, attend, document minutes and follow up on action items of inter-professional and/or committee conferences and other Cancer Institute specific programmatic meeting structures. · Collect and maintain raw program quality metrics by way of chart extraction and physician collaboration. Complete quarterly audits of patients and prepare presentations. · Support & facilitate patient education/community education sessions and support groups. Assist in maintaining patient and referring provider educational materials and patient collateral. · Communicate with patients and distribute unique, diagnosis specific, material (to assess prior studies. Track patient throughput and document in patient’s electronic health records and communicate with providers, nurses, medical assistants, genetic counselors, and other care team members as needed. · Ensure CME requirements for various providers and practitioners are maintained. · Prepare for accreditation site visits. · Prepare and design PowerpPoint presentations, draft agendas, internal communications, and other necessary materials. · Ability to travel to multiple sites across healthcare system *Education: · Bachelor’s Degree Required Experience: · Two (2) years of experience in a health-related field or project/data management with oncology experience strongly preferred We take great care of careers.
