Spring Creek Rehabilitation and Nursing Center is currently looking for an internal, facility based, full time Nurse Practitioner (CRNP)! Must be knowledgeable of and demonstrate the ability to provide advanced clinical skills to assess/manage the healthcare of chronically ill geriatric residents in a 404 bed Skilled Nursing Facility. Responsibilities of the Nurse Practitioner (CRNP):

  • Performing health histories and in-depth medical exams for new residents, to determine preliminary Plans of Care

  • Initiating, evaluating and reviewing medical orders, and identifying opportunities to improve care, when possible

  • Carrying out various procedures and documenting all care and recommendations

  • Ordering, interpreting and following up on lab tests and results

  • Requesting consultations from medical and non-medical providers, including physicians, therapists, dietitians, social workers and other specialists

  • Educating staff, residents, caregivers and families on essential care functions to ensure residents have maximum quality of life in our facility

  • Completing ongoing assessments to determine Plan of Care changes or improvements based on resident feedback and medical evaluations

Requirements of the Nurse Practitioner (CRNP):

  • Must be a licensed CRNP from an accredited university and participate in continuing education/workshops for license retainment

  • Must have an active Prescriptive Authority

  • Minimum of 1 year experience working with the frail, elderly population

  • Demonstrate competency to treat geriatric patients

  • Able to solve problems, think analytically, work well with an Interdisciplinary Team, and have good communication skills

  • Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.
