Highlights
Community Health Network is seeking a Psychiatric-Mental Health Nurse Practitioner (PMHNP) to join the Community Behavioral Health team in the Community Mental Health Center setting. This provider will practice within Community’s outpatient clinic located in Greenfield, Indiana, and will work with both adult and youth populations. This position also entails providing medication-assisted treatment (MAT). Ideal candidates will be highly accountable, independent, and have a desire to deliver direct, collaborative patient care. A minimum of 2 years' provider experience is strongly preferred.

Licensure & Certification
