Care Delivery Coordinator (Scheduler)  ROLE PURPOSE: Responsible for the operational support of the Caregiver Teams to ensure  services provided to homebound patients in their place of residence is the highest quality of care.  Assists the Care Delivery Team with managing resources so that quality standards are maintained,  and the continuity of care is uninterrupted.  RESPONSIBILITIES:  • Participates in On-Call / After-Hours duties in conjunction with the Care Delivery  Manager to maintain consistent, timely and uninterrupted service. • Provides support to Director of Clinical Services and Care Delivery Manager in  documenting and resolving Caregiver concerns, feedback, or incidents. • Participates in the management of ongoing Caregiver and Patient compatibility along  with the reallocation of Caregivers from existing visits or cases. • Participates in the resolution of Caregiver scheduling/shift challenges along with the  reallocation of Caregivers from existing cases or those in On-Call status. • Involved in the recruiting, screening, interviewing, and onboarding of new Caregivers. • Works closely with all management personnel to maintain effectiveness of the Care  Delivery operations. CRITICAL DEMANDS:  1. Capacity to assist with caregiving in emergency situations and on-call scheduling after  normal business hours/weekends. 2. Ability to deal effectively with stress while multi-tasking and adapting to different  situations 3. Completes schedules, communications, and documentation with accuracy and within  agency time frames. 4. Requires minimal supervision to be successful and is self-directed when assessing  priorities. ESSENTIAL SKILLS:  • Strong phone and computer skills along with problem-solving and decision-making abilities in a fast-paced environment. • Maintains stable performance and emotions when faced with oppositions, pressure  and/or stressful conditions. • Exhibits adaptability, flexibility, self-control and maturity in work and behavior. • Utilizes listening skills that indicate understanding and promotes accurate interpretation  of others' concerns, motivations and feelings.
