Job Description
Assesses and/or interviews patients for admission to in/outpatient rehabilitation. Consults and collaborates with discharge planners, social workers and medical staff. Admitting policies and procedures. Develops strategies to increase visibility in the community, determines expeditious admissions and serves as a liaison for referrals.
Job Responsibility
Establishes and develops community/ physician referrals for possible admissions.
Visits referring facilities on a daily basis to obtain patients for admissions.
Serves as liaison among community discharge planners, MD's and Rehab admissions.
Obtains and screens clinical information.
Communicates and coordinates special needs of the patient for admission.
Assesses and screens residents prior to admission.
Receives requests for admission from a variety of referral sources.
Performs admission assessments evidenced by appropriate documentation from the patient medical record.
Obtains pertinent clinical and demographic/insurance information; completes appropriate pre-admission forms.

Job Qualification
High School Diploma or equivalent required.
1-3 years of relevant experience, required.

The salary range for this position is $36,980-$55,690/year
