Director of Case Management

Job Description

The Director of Case Management is responsible for managing and coordinating department activities, ensuring staff compliance with organizational policies and external regulatory agencies. The role involves coordinating the integration of patient care and discharge planning processes with related hospital departments and external agencies, ensuring continuity of care and optimal clinical resource utilization. This position requires an executive presence with excellent communication and presentation skills, proven motivational capabilities, and a demonstrated record of delivering results and achieving goals.

Responsibilities

  • Direct and evaluate departmental operations, including the case management model, staffing, use of information technologies, onboarding, and staff competencies to achieve performance and quality objectives.

  • Allocate resources effectively to staff department and meet productivity and quality goals.

  • Oversee case management core functions and practices.

  • Ensure adherence to care coordination and discharge planning processes, ensuring timeliness, quality, and proper documentation.

  • Collaborate with the Facility CFO and Division CM Leader to achieve established goals and expectations.

  • Assess and improve department performance by evaluating operational processes, monitoring performance through data analysis, and implementing sustainable performance improvement activities.

  • Ensure compliance with policies and standard operating procedures.

  • Establish working relationships with key stakeholders including CMO, CNO, ancillary service leaders, Ethics & Compliance, and Legal.

  • Perform other duties as assigned.

  • Practice and adhere to the Code of Conduct and Mission and Value Statement.

Essential Skills

  • At least 2 years of recent acute care case management experience and 5-7 years of clinical experience.

  • Bachelor's degree in a relevant field.

  • Registered Nurse (RN) licensure.

  • Proficiency in case management and care management.

  • Strong supervisory skills.

  • Knowledge of medical terminology and health information management.

Additional Skills & Qualifications

  • Master's degree in Nursing, Health Administration, or Business Administration preferred.

  • Experience with Epic systems.

  • Ability to manage multiple priorities with minimal supervision.

  • Demonstrated leadership, trust building, team building, and change management skills.

Work Environment

The work environment is located at the Oviedo Medical Center campus, which includes a 64-bed hospital, a 22-bed emergency department, and an adjacent medical office building. The center is equipped with advanced medical technology and staffed by dedicated professionals offering a full range of medical services. The hospital treated 3,151 patients in 2020 and employs 420 staff members, including 170 physicians. The campus supports a collaborative and dynamic work environment focused on quality patient care.

Pay and Benefits

The pay range for this position is $100000.00 - $130000.00/yr.
