Salud Family Health Centers WiseWoman Patient Navigator in Fort Lupton, Colorado

WiseWoman Patient Navigator As a grantee of the Colorado Department of Public Health and Environment (CDPHE), Salud is able to offer the WiseWoman (WiWo) Program to eligible patients for integrated care for cardiovascular disease prevention. The WiseWoman Patient Navigator functions as a member of the patient care team performing patient navigation for both insured and uninsured women through the Health Navigation and Clinical Services component of the WiseWoman Program. The WiseWoman Patient Navigator approaches his/her tasks in a team-based care fashion and provides support and guidance to patients throughout the cardiovascular screening process, risk reduction, and health coaching.

We offer: Starting Wage - $18.38 to $21.13 per hr. Monday to Friday - Available 7 am to 4 pm

Qualifications

Education: High school diploma or GED, required

Experience: One to two years of clinical work along with working with a diverse and/or underserved population in a health setting preferred Experience providing patient health education or health coaching to reduce cardiovascular disease is preferred

Licensure: None

Knowledge, Skills, and Abilities: Able to relate well to patients and co-workers Possess sensitivity toward the needs of others Must be a self-motivated, proactive, and independent worker Possess excellent organizational skills Must work well with the patient care team Bilingual English/Spanish preferred Sensitivity to low-income and ethnic minority communities is a must
