Full-Time Family Health Worker - Dental - Queens

Responsibilities :

• Responsible for treatment planning and care coordination

• Take care of insurance verification and issue resolution

• Review and attachment of documents for claim submission

• Handle updating of forms for compliance, i.e., translation updates, patient education, etc.

• Manage and track patient referrals

• Facilitate urgent referral for patients

• Direct patients to appropriate specialist referral

• Assist in obtaining consultation/specialists reports back to the health center

• Ensure that the patients/families have adequate understanding of referral procedures and the reason for specialist referrals

• Provide education and support to patients/families about the importance of keeping the specialist referral appointments

• Assist staff with orientation and training of volunteers and trainees

• Assist patients with scheduling appointments with specialists

• Collaborate with social workers to provide assistance and support to patient with financial or health insurance issues

• Act as the point of contact for patients/families with issues related to referrals

• Generate weekly referral and test tracking EDR reports

• Follow up with patients post-specialist visits

• Perform other duties as assigned

  Minimum Qualifications :

• Two years community college or equivalent

• One to two years community health or health care experience

• Bilingual in English/Chinese highly preferred

  Compensation and Benefits :

• Salary: $40,000.00 - 46,000.00 per year
