The Clinic Population Health Lead will work in collaboration with primary care providers and healthcare teams to assist in coordinating, facilitating, monitoring and evaluation clinically complex patient populations to ensure quality of care and cost-effective use of resources consistent with the MercyOne Elkader Medical Center mission.

What you will do:

  • Works collaboratively with ACO to identify Medicare patients appropriate for Annual Wellness Visits (AWV) and execute guided processes to complete the visit aligned with quality standards.

  • Attend and actively participate in ACO population health related trainings and roadmap teleconferences as required.

  • Establish competency with a variety of assessments related to the AWV and assist in training clinic team members to conduct these assessments.

  • Assist and coach patients with self-management of their chronic diseases and lifestyle changes to mitigate health risks.

  • Collect pertinent patient health care data, review medical records, perform interventions and confer/collaborate with physician/mid-level providers to ensure patient needs are met.

  • Communicate effectively with patients, co-workers, and others. Promote wellness and provide patient education.

  • Maintain an office environment that is safe, neat, organized, and supplied with necessary equipment, instruments, supplies, and educational materials. Ensure equipment is functioning safely and properly. Perform cleaning and maintenance of equipment according to guidelines.

  • Demonstrates flexibility in performing other additional duties of patient care. Provide input for devising systems to improve quality and efficiency of patient care and workflow.

  • Use and operate a wide variety of diagnostic and medical equipment.

  • Adapting to fill schedule needs where they arise to accommodate needs of clinic operations.

  • Other duties as assigned by Clinic Manager.

What we are looking for:

  • Must be a licensed practical nurse (L.P.N.) and must hold an active license to practice nursing in the State of Iowa.

  • Previous experience in caring for patients with chronic disease (required).

  • Previous Care Coordination, Case Management, Home Health or Behavior Health experience (preferred).

  • One year experience in provider practice or clinic health setting (preferred).

  • Perform duties within scope of licensure, act as a resource for department staff regarding duties and problem solving.

  • Demonstrate competency in compliance of MercyOne Elkader Medical Center policies, procedures, and guidelines.

  • Maintain patient confidentiality, work cooperatively to foster a positive work environment within MercyOne Elkader Medical Center, MercyOne Elkader Family Medicine, MercyOne Monona Family Medicine and other departments, as well as with its clients.

  • Provide quality healthcare to clinic patients and assure patients’ needs are met, serving as a healthcare advocate for patients.

  • Utilize time management skills to effectively prioritize tasks.

  • Maintain infection control and safety guidelines.
