New York City Health + Hospitals Correctional Health Services CCS Case Manager in EAST ELMHURST, New York
Greet patients newly admitted to NYC jail system Obtain voluntary patient consents (e.g., HIPAA) Scan and upload documentation to patient's chart in the electronic health record Maneuver within the jail-based clinics and/or housing areas to complete services Additional tasks and projects as requested by CHS/RATS leadership Comfortable engaging patients in a correctional setting Ability to manage tasks independently Great time management skills Experience with the criminal, legal. and social service systems Excellent interpersonal and communication skills Basic computer skills to scan, organize and access electronic health records Knowledge of benefit entitlements, particularly Medicaid 1. A four-year high school diploma or its educational equivalent and four (4) years of satisfactory full-time experience providing direct counseling, guidance, crisis-intervention or information and referral services for inmates of a correctional facility, substance abusers or a similar client population; or 2. Thirty (30) semester credits from an accredited college will be considered equivalent to one (1) year of the required experience. However, all candidates must have a four-year high school diploma or its educational equivalent and at least one (1) year of experience as described in "1" above.
