Job Summary

This Advanced Practice Provider (APP) will predominantly support the Denver Prevention Training Center's STI Regional Center utilizing skills to design, develop, and deliver clinical consultation, training, technical assistance, coaching, and advanced facilitation of activities to enhance STI, HIV, and syndemic prevention, care and treatment services in a multi-state region. The APP also works collaboratively with a supervising physician to provide direct clinical care for patients presenting to the Denver Sexual Health Clinic. The APP works in close communication with the unit's multidisciplinary team which includes social work, care management, medical assistants, nursing, research, pharmacy and others to help coordinate comprehensive care for their patients.

Essential Functions:

Delivers superior patient care and encourages increased patient understanding and involvement in care. Determines and assesses the health status of patients by reviewing records, taking comprehensive medical histories, performing physical examinations, ordering appropriate lab tests, making diagnostic and treatment decisions, and consulting with attending MD according to clinic protocols. (10%) Monitors and evaluates medical condition of patient. (5%) Initiates, performs, or directs other health care professionals to perform screenings and diagnostic tests. Makes decisions regarding treatment changes and refers patient to specialized services as needed. Orders and administers appropriate medications within the limits of the appropriate statutory limitations. (10%) Completes required documentation legibly, thoroughly, and in a timely manner. Prepares and maintains complete and accurate charts, case files, and records. (5%) Participates in clinical education of health care students as seen fit by the supervising medical staff. (5%) May be assigned to perform lead work and/or involvement in clinical coordination, health station management or responsibility for special projects which may encompass protocol development, management planning, quality improvement activities, scheduling and other administrative tasks as necessary to support the practice environment. (5%) Leads organizational readiness assessments including collating information, creating short-term and long-term objectives, and determining monitoring strategies with clinic and organizational leadership. Designs technical assistance and training plans to meet organizational needs. (20%) Trains clinical professionals on various topics and curricula including, but not limited to, topics related to HIV/STI diagnosis, treatment, and management; prevention in care; syndemics, and behavioral interventions used in care environments using a racial health equity lens. Coordinates the STI Intensive course. (20%) Coordinates the Nursing Continuing Professional Development (NCPD) program, including developing applications, maintaining compliance, and completing required reporting for the Denver PTC. (10%) Coordinates the STD Clinical Consultation Network (STD CCN) and provides clinical consultation to clinical providers in a multi-state region. (10%) Education:

Master's Degree Required Work Experience:

1-3 years Related clinical experience preferred Licenses:

BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association Required Knowledge, Skills and Abilities:

Knowledge of and ability to apply professional medical principles, procedures, and techniques. Skill in eliciting history from patients, performi g physical exams, formulating assessments, and determining treatment plans for patients. Experience ordering appropriate diagnostic studies to aid in diagnosis. Knowledge of appropriate administration and prescription of medications. Ability to exercise initiative and sound judgment. Ability to respond appropriately in emergency situations. Ability to prepare accurate documentation and reports. Ability to establish and maintain effective working relationships. Proficiency in Microsoft Office Suite is required. EPIC experience preferred.
