Job Description:

Provides person-focused therapeutic services for individuals experiencing mental and behavioral health events of varying acuity, inclusive of substance use and/or withdrawal. Contributes to the collaborative plan of care and delivery of services to individuals through assessment, evaluation, crisis interventions, referrals, education, linkage to care, and progress toward treatment plan outcome-based goals. Works as part of an interdisciplinary team with the individual as decision-maker.

Essential Functions:

  1. Work collaboratively with colleagues of the mobile servicesas well as community partners, providing support to individuals through the crisis and substance use recovery process.
  2. Work with individuals in crisis to develop a crisis plan, provide linkages to care, and introduce other crisis planning tools and professionals as appropriate and as needed, prior to the conclusion of the first engagement.
  3. Work with all members of the team to facilitate engagement of the individual who is in crisis; helping them feel welcome and comfortable.
  4. Work with individuals in crisis to assist with continuity of care by supporting connection to behavioral health, addiction, peer recovery, and other community resources.
  5. Builds relationships with addiction, behavioral health, peer recovery, community outreach providers supporting vulnerable populations, and other community resources.
  6. Performs timely and accurate documentation of all individual interactions for support, education, and advocacy, including clinically related information.
  7. Empower individuals to become self-advocates for their own recovery and wellness.
  8. Works with all individuals using a strengths-based perspective, providing support and encouragement.
  9. Coordinate and act as a liaison as assigned and appropriate for internal and external stakeholders.
  10. Demonstrate adherence to company policies and expectations and behavioral competencies, both explicit and implicit.
  11. All tasks, projects, and/or duties as assigned and authorized, as determined by Human Resources and Direct Supervisor (or designated delegate) to be congruent with the role.
Qualifications

Education:

High School Diploma, GED or Equivalent, Required

Associate Degree in Human Services or related field, Preferred

Experience:

Behavioral health and/or addiction lived experience, Required

Knowledge of county behavioral health supports, Strongly Preferred

Additional:

Valid and unrestricted drivers license, Required

Proficiency with Microsoft Office Suite Products, Required

Proficiency and familiarity with EHR/EMR systems, Preferred

Credentials:

Current BLS Certification, Preferred

Physical Demands:

Prolonged periods of sitting at a desk or computer workstation.

Prolonged periods of standing, bending, and reaching.

Must be able to lift weights up to 50 pounds at times.
