Clinical Medical Social Worker - School-Based Health Service Requisition ID 2024-49119 Category Social Work Overview School-based Health Services increases access to health care at multiple Central Ohio Schools, providing primary care and a variety of other services to students and families. The Clinical Medical Social Worker embedded in the School Based Health Clinics are a part of a multidisciplinary team to improve health outcomes. They promote health and well-being by providing direct support and services to students and families within the clinics. The social worker also supports families with navigation of health care, health education, referral and linkage to community resources, and collaboration with school personnel and community partners. The social worker is also responsible for assessment of behavioral health needs and safety planning, social determinants of health, and child maltreatment. Provides developmentally appropriate, outcome-focused diagnostic and therapeutic service to patients and families in accordance with the scope of practice as defined in Chapter 4757 of the Ohio Revised Code, Section 4757.01 et seq. Practices professional social work and promotes advocacy in accordance with the NASW Code of Ethics for Social Work and maintains a current Ohio Social Work license. Responsibilities Functions as a member of the interdisciplinary team and provides Clinical Medical Social Work services to patients and their families to improve or maintain social, emotional, functional and physical health (Continuously). Provides consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services (Frequently). Participates in the development and delivery of education, research and continuous quality improvement (Frequently).
