Ohio Means Jobs-Cuyahoga/Cleveland County is sourcing for a case manger to work in downtown Cleveland. Job Summary: Support local families on their journey from crisis to stability! Interview, assess, collect client data, collaborate with various Teams and community partners to formulate a personalized plan of action for client families. Must be able to work with clients who are involved with other social service systems, mental health, and adults in crisis. Essential Duties and Responsibilities: · Must be conversant in all social services methodology. · Work as a team member with local Social Services staff to select viable client families.. · Work closely to form an interdisciplinary team to empower family clients to move from crisis to stability. · Maintain a caseload of at least 16 active families, starting in the sixth month of employment. Seek out new families when graduating or exiting current families. Communicate with Regional Coordinator if caseload expectation is not maintained for over one month. · Maintain weekly contact with families and log case/care management and all client interactions · Maintain all statistical reports, case notes, and client assessments as pertains  · Work with Corps Officer/Corps Ministry Teams to encourage client families to participate in the holistic program of The Salvation Army. · Process Pathway of Hope forms, i.e., recording and maintaining client files according to Salvation Army standards. · Must familiarize self with community resources and existing, as well as, potential collaborative partners. · Attend meetings and service provider agency meetings as requested · Network with service provider agencies. · Strictly adhere to company policies pertaining to confidentiality, release of information, and retention and/or destruction of records · Work closely with, Divisional Director, and the Regional Coordinator. · The above duties and responsibilities are not all inclusive and may be modified to include other job-related duties at the discretion of the supervisor. Knowledge, Skills and Abilities: · Excellent communication and listening skills. · Ability to work on multiple projects and prioritize accordingly. · Able to interact and communicate effectively with team members, clients and agencies. · Knowledge of Microsoft computer applications. · Knowledge of client management and database systems. · Flexibility and good organizational skills. · Must have ability to adapt to various work locations and settings. · High level of initiative and ability to work independently. · Must be familiar with social work ethics. · Must have good time management skills. · Must have a valid OH driver license and vehicle. · Respect of cultural differences, levels of education, physical functioning, and crisis situation in a non-judgmental manner. Education and Experience: · Bachelor's Degree of Social Work or related discipline required or five years of related experience. · All three OH Clearances required upon employment and thereafter upon renewal date: OH Child Abuse History Clearance. Request for Criminal Record and Fingerprint based federal criminal history. Clean Ohio clearance. · Mandated and Permissive Reporting  · Proficiency in Spanish preferred but not required. Hours, Salary and Benefits: Monday thru Friday 8:30am-4pm, Pay range $18-$22 per hour, Dental, Health Insurance, Paid Holidays, retirement, sick leave and vacation.
