he Care Associate (M) is an active, member of the care team in Continuing Care who is committed to providing care to the residents of the neighborhood to meet their daily needs and enable them to function at the highest possible level.

Compensation: Up to $23.00 per hour with shift differential; rate is based upon years of licensure.

The Care Associate (M) provides care to the residents of the neighborhood to meet their daily needs and enable them to function at the highest possible level to include:

  • Provides physical assistance with activities of daily living

  • Promotes psychosocial well-being through meaningful relationships

  • Plans and facilitates social activities for the neighborhood residents

  • Assists with menu/snack selection and partners with Dining in serving to ensure adequate nutritional intake of all residents. Provides in-room dining experiences where requested.

  • Assures the resident’s room and neighborhood are kept tidy to minimize environmental hazards which may impact environmental safety

ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.

Provides physical assistance with activities of daily living (ADL) and Independent Activity of Daily Living (IADL) to include but not limited to dressing, personal hygiene, dining, mobility/transferring, light housekeeping, laundry or other home chores. Supports and promotes mental well-being through meaningful relationships and interactions Collaborates/partners with Programming team and facilitates social activities for the neighborhood residents Assists & collaborates with Dining team with meal/snack selection to provide adequate nutritional intake to residents per individualized Holistic Service Plan. Provides in-room dining experiences as instructed or requested. Effectively implements individualized Holistic Service plans to residents on neighborhoods. Responsible for maintaining required job skills and knowledge per state regulations and/or Erickson standards. Responsible for promptly notifying Assisted Living/Memory Care Manager promptly of any change in a resident’s condition. Works with other neighborhood staff members to engage the resident in activities of daily living and maintaining the functioning of the neighborhood (i.e., laundry, cooking, meal planning/preparation/serving/cleanup, etc.). Assist group and individual activities (structured and impromptu) in activity spaces, common areas and in the resident’s room to support resident desires. Maintains equipment, games and supplies for use in supporting resident and neighborhood activity choices. Attend all required training as needed to State regulations and/or Erickson standards.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Basic computer skills required.

EDUCATION and/or EXPERIENCE:

  • Must be at least 18 years of age

  • Certified Nursing Assistant/Certified Home Health Aide experience is preferred.

  • Current CPR certification preferred.

LANGUAGE SKILLS:

Must be able to read, write, understand and communicate in the English language.

LICENSES, CERTIFICATES, REGISTRATIONS:

  • Current certification from the State Board of Nursing is required Certified Nursing Assistant, Certified Home Health Aide preferred

  • Successful completion Certified Nursing Aide training and awaiting to take certification tests (Nursing Assistant)
