JOB DESCRIPTION

Position: Direct Support Professional Job Mentor FLSA: Non-Exempt

Reports To: Support Coordinator Program: MBSI, MBCE, MBOE

Summary: Serve as instructor, mentor, and advocate for individuals on assigned caseload. Assure all services contained in person centered plan are provided either directly or in coordination with other Support Professionals. Collect program data relative to services and outcomes. Complete program notes and reports and recommend plan goals or modification. Responsible for the safety, quality of service, satisfaction and care of the person served.

Essential Job Functions:

Provide work and employment training services training to people who are cognitively, behaviorally and/or physically disabled.

Travel to various worksites in the area and provide job support to clients at their place of employment. Job supports may include shadowing, modeling, or assisting with job duties.

Provide or arrange transportation for clients if required.

Monitor progress towards goals, collect data on all services provided, evaluate results, make recommendations.

Complete documentation and maintain accurate records, prepare reports as required.

Assess the strengths of individuals and teach them to use them effectively.

Teach job skills to the employee and guide in learning to complete job tasks.

Assist the employee with soft skills such as social skills and communication in the workplace.

Counsel employee on fine-tuning work habits/skills to maintain employment.

Help the employee stay motivated through difficult times.

Encourage the development of self-esteem and independence.

Work with the employer to identify their needs and workplace goals, and the employees role in them.

Communicate and coordinate with other service providers to ensure that goals, supports, and services provide satisfactory outcomes.

Provide for the safety of all persons served. Ensure vehicles are operated and maintained in a clean, safe, and healthy manner.

Attend and actively participate in meetings as required.

Adhere to all Witco standards, policies and procedures.

Participate in fund raising events sponsored by Witco.

Assumes responsibility for other duties as required or assigned by Support Coordinator.

Education/Experience:

High School Diploma or GED.

Basic computer operating skills

Requirements and Skills:

Pre-employment drug screen and criminal history background clearance.

Valid drivers license, proof of insurance and an excellent driving record is required.
