Ultimate Care Inc Home Care Liaison in Brooklyn, New York

Ultimate Care NY, a leading provider of home health care services, is seeking Home Care Liaison to join our team!

The Home Care Liaison will play a key role in building and maintaining relationships with healthcare providers, case managers, social workers, patients, and families to promote the services of Ultimate Care NY. This role involves coordinating care, ensuring client satisfaction, and increasing the referral base by effectively communicating the benefits of home care services. The Home Care Liaison will work closely with the Sales and Marketing Director to develop and implement strategic initiatives to achieve business goals.

Work Environment:

  • This position involves frequent travel within the community (NYC) and to healthcare facilities.

  • The role may require flexible hours, including evenings and weekends, to meet the needs of clients and referral sources.

Compensation: $60,000- $70,000 annually

You will:

Relationship Building:

  • Develop and maintain strong relationships with referral sources, including hospitals, physicians, skilled nursing facilities, case managers, social workers, and other healthcare professionals.

  • Build and maintain relationships with case managers and social workers to facilitate referrals and ensure the smooth coordination of care.

  • Educate healthcare providers, case managers, social workers, and community organizations about the benefits and services offered by Ultimate Care NY to foster collaboration and trust.

  • Act as the primary point of contact for clients and their families, ensuring they receive excellent service and support.

Marketing and Outreach:

  • Promote Ultimate Care NY's services through regular visits, presentations, and networking events.

  • Identify and pursue new business opportunities and referral sources.

Client Coordination:

  • Coordinate with the clinical team to ensure seamless service delivery for new clients.

  • Conduct initial assessments and follow-up visits to evaluate client satisfaction and address any concerns.

Sales Strategy:

  • Collaborate with the Sales and Marketing Director to develop and execute sales strategies and marketing plans.

  • Track and report on referral sources, client acquisition, and retention metrics.

Administrative Duties:

  • Maintain accurate records of client interactions, referrals, and marketing activities.

  • Prepare reports and presentations for management as required.

You will have:

  • Bachelor’s degree in healthcare administration, marketing, business, or a related field.

  • Minimum of 2-3 years of experience in healthcare sales, marketing, or a related field, preferably in home care.

  • Strong interpersonal and communication skills.

  • Proven ability to build and maintain professional relationships.

  • Excellent organizational and time management skills.

  • Knowledge of home care services and regulations.

  • Valid driver’s license and reliable transportation.
