RiseBoro Homecare Inc. Case Assistant, Diana Jones SC in BROOKLYN, New York
ESSENTIAL DUTIES AND RESPONSIBILITIES
Under the general supervision of the Director, the Case Assistant will provide individualized case assistance, resources, and referrals to members of the Diana Jones Senior Center and seniors in the surrounding community. Specific duties and responsibilities include:
Processing membership enrollment including data entry into funding source required database
Provide information, entitlement, and case assistance to seniors
Maintain up-to-date files on client entitlements and benefits
Compile case assistance logs as required by the program funding source
Maintain electronic and physical data/documentation for all clients who require social services, benefits, and/or entitlements; refer to the appropriate service providers (internal and external) for assistance
Assist with educational/recreational activities, special events, and trips as needed
Attend relevant training to stay up to date on resources available to senior citizens
Compile statistics, prepare reports, track attendance, and maintain other records of information as needed
Represent RiseBoro in community and interagency activities
Other duties as required by the Director

Knowledge of funding source databases i.e., PeerPlace preferred
Mental Health First Aid skills-based training course completion a plus
Proficient in Microsoft Office Suite, particularly Word, and Excel
Ability to understand the concepts of institutional and structural racism and bias and their impact on underserved and underrepresented communities
Commitment to supporting communities who have experienced systemic oppression and bias (e.g. people of color, LGBTQ+ people, immigrants, justice-involved persons, etc.)
EDUCATION EXPERIENCE
(BSW) Bachelor's degree in Social Work preferred
Bachelor’s degree in a related field such as but not limited to human services, counseling, sociology, psychology, gerontology, etc., with a minimum of one (1) year of human service experience including (internship and volunteer experience)
Associate degree with a minimum of one (1) year of direct experience in case assistance (including internship and volunteer experience)
High School degree or equivalent (HSE/GED) with at least one (1) year of direct experience in case assistance and one (1) year of related experience or training in customer service, health and human services (including internship and volunteer experience)
