TITLE: Case Worker TYPE: Exempt Non-Exempt SUPERVISOR: Program Director & Assistant Director POSITIONS SUPERVISED: N/A

Position Summary: Determine client needs and eligibility for assistance, arrange for services. Work under the guidance of the supervisor, or director

Essential Duties and Responsibilities: - Provide direct services to clients, including intake, referrals, benefits and entitlements assistance, advocacy, counseling and follow-up. - Maintain and update client's files and ensure that all information is documented, and all contractually required documents are maintained in files. - Responsible for knowledge and compliance of DFTA's case assistance and information/referral standards. - Fill out entitlements/benefits applications such as SNAP, Medicaid, Medicare Savings Program, Senior Citizen Rent Increase Exemption (SCRIE), Home Energy Assistance Program, NYC-210, IT-214, Housing applications, etc. - Arrange for provision of services, advocate and intervene with other agencies on behalf of client. - Responsible for the submission of case assistance logs on a daily basis. - Maintain calendar and schedule appointments. - Assist with review and explanation of mail. - Supportive contact with clients awaiting decision on eligibility for entitlements/benefits. - Compile statistics and maintain up to date records on each case. - Attend In-service trainings. - Perform additional duties as assigned by supervisor or director.

Minimum Qualifications: - Associate's degree or higher, with two years' supervised experience in human service field. - Knowledge of benefits and entitlements for seniors. - Bilingual English/Spanish speaking strongly preferred.
