Vibrant Emotional Health Case Planner in Bronx, New York

Position Title: Case Planner

Department: Family Connections

Reports to: Program Supervisor

Location: Flexible Hybrid, Field work in the Bronx required

Schedule: 9:00am – 5:00pm /Evening hours as necessary to accommodate working families

Hourly Pay Range: $31.32 - $ 35.71*

*New hires are typically brought into the organization between the minimum to midpoint of the salary range posted depending on qualifications, internal equity, and the budgeted amount for the role.

Position Overview:

The Family Link Preventive Programs are in need of a Bilingual Case Planner, who will work with families in the Bronx to support and strengthen their ability to nurture their children in a safe and secure environment.

The Case Planner will work with a team who collaborates with families to develop an individualized service plan and connect them to the specific services they may need including supportive counseling, parenting classes and support groups, mental health and substance use treatment, housing advocacy, financial literacy, immigration support, and support navigating social service systems.

Job Duties & Responsibilities:

  • Provide clinical services, families’ homes and in office on a weekly or bi-weekly basis

  • Support 12 enrolled families with higher caseloads as needed in times of crisis

  • Perform intake and case management functions, including crisis response services

  • Collaborate with families to assess needs and create goals that support families in change intervention

  • Collaborate with Family Advocate to ensure positive communication and cooperative work

  • Attend and participate in Family Team Conferences.

  • Attend and participate in team meetings to maintain goal alignment through supervisory and peer support

  • Undertake activities that promote advocacy and referral to services

  • Produce and maintain documentation in compliance with policy and best practices

  • Attend 42 hours of job-related in-service training sessions annually

Qualifications:

  • BA or BSW required

  • Bilingual and bicultural candidates are encouraged to apply, Spanish preferred

  • An interest in or experience with motivational interviewing

  • Ability to interact with a multicultural team

  • Strong ability to conduct presentations to staff and families

  • Excellent time management skills

  • Excellent written and verbal communication skills

  • Excellent analytical, organizational and interpersonal skills

Physical and Sensory Requirements:

  • Ability to stand for extended periods of time

  • Ability to travel and conduct field work
