State of Massachusetts Clinical Social Worker (C) in Boston, Massachusetts

The Dr. Solomon Carter Fuller Mental Health Center (DSCFMHC) is seeking an experienced and professional Clinical Social Worker C (CSW C).* * The CSW C will Performs biopsychosocial diagnostic assessments by conducting interviews of patients and their families / significant others (if appropriate), contacting outpatients service providers, and investigating histories including contacting prior agencies, schools, court personnel, and other sources of collateral information in order to identify strengths and needs, and to develop a treatment plan which maximizes patient functioning in the least restrictive setting. Duties and Responsibilities (these duties are a general summary and not all inclusive): * Completes a comprehensive written biopsychosocial assessment which is maintained in the patient’s electronic medical record within five days of admission of the patient according to established MHIS workflow. * Coordinates the development of individual treatment plans for patients on caseload in order to ensure that the multidisciplinary team has completed a Comprehensive Treatment Plan according to established criteria and timelines. * Coordinates Periodic Reviews for assigned cases, and ensures the Periodic Reviews are completed by the treatment team according to established timeframes and procedures. * Provides clinical case management for cases assigned in order to ensure timely and effective delivery and management of services identified on the Comprehensive Treatment Plan. * Coordinates pre-discharge arrangements and communications, completes oral and written referrals in an accurate and timely manner to aftercare services including DMH eligibility determinations, referrals for psychosocial rehabilitation services, outpatient therapy and medication, residential services, substance abuse services, medical services, and other agencies as determined by patient need. * Provides skilled clinical social work services to patients and their families, utilizing a wide array of intervention modalities including individual, group, and family therapy for the purposes of: changing maladaptive behavior, enhancing client capacity to cope with symptoms of mental illness and / or substance abuse, and improving communication and interactions between the client and his or her primary support system. * Provides highly skilled psychoeducation services to patients, their families, and other service providers in order to increase understanding of mental illness, substance abuse, legal factors, and other issues as determined by patient need in order to enhance patient and family knowledge and utilization of rehabilitation processes and resources. * Documents interventions in progress notes, group treatment notes, and MHIS assessment documents according to established standards. * Leads and/or co-leads at least one inpatient group per week. * Documents discharge plan as a component of weekly progress note. * Completes financial assessments including assessment of patient capacity to manage funds, patient entitlement and insurance benefits. Refers identified needs to financial benefits coordinator, maintains responsibility to ensure that appropriate applications are made for entitlement benefits. * Maintains knowledge of current developments in statutes, case law, and departmental / agency regulations in the fields of social work, mental health, and forensic mental health. Utilizes this knowledge in providing direct services and in collaboration with multidisciplinary care providers. * Reviews forensic caseload weekly to ensure that all legal coordination is implemented as needed in a timely manner. * May provide supervision and leadership within the Social Work Department and the facility. * Participates in agency and department performance improvement / quality improvement activities. * Attends seminars, workshops, conferences, and staff meetings to maintain professional proficiencies, develop clinical competencies, and maintain professional license. * Is able to demonstrate the knowledge and skills necessary to provide care to adult and geriatric patients. * Performs other duties as assigned by the Director of Social Work consistent with professional standards, the goals, and objectives of the program and with the policies of Dr. Solomon Carter Fuller Mental Health Center and DMH. Required Qualifications: * Demonstrated knowledge of the principles and practices of psychiatric and basic social work practices, including but not limited to, substance abuse treatment, outpatient care to people with severe mental illness, physiological, psychotherapies, psychological, and emotional growth, and development. * Familiarity with the types and applications of emergency medical procedures, such as CPR. * Basic uses of diagnostic procedures employed in social work practice. * Knowledge of interviewing and problem-solving techniques. * Ability to follow and give clear and precise oral and written instructions. * Capacity to participate in a multi-disciplinary health care team. * Skilled in preparing and implementing treatment plans. Preferred Qualifications: * Experience working with young adults with various physiological or psychological needs. * Awareness of various community agencies and the resources they provide and coordinate (either directly or via other service providers) referrals to appropriate resources for clients based on need. * Capacity to undertake multiple assignments and adjust priorities as needed. * Excellent interpersonal skills and the ability to communicate effectively with diverse populations. * Ability to provide guidance and consultation to a multidisciplinary team. * Proficiency in Microsoft applications such as Word, Outlook, and PowerPoint. * Given the population served bilingual, or multilingual fluency. Please Note: *This position is full-time, 40 hours per week. The Schedule is 8:30am to 5:00pm, Monday through Friday. MINIMUM ENTRANCE REQUIREMENTS: REQUIRED WORK EXPERIENCE: At least two years of full-time, or equivalent part-time, professional experience as a clinical social worker after earning a Master's degree in social work. SUBSTITUTIONS: - A Doctorate in social work, psychology, sociology, counseling, counseling education, or human services may be substituted for the required experience on the basis of two years of education for one year of experience. - One year of education equals 30 semester hours. Education toward a degree will be prorated on the basis of the proportion of the requirements actually completed. REQUIRED EDUCATION: A Master's or higher degree in social work is required. LICENSES: - Licensure as a Licensed Certified Social Worker or Licensed Independent Clinical Social Worker by the Massachusetts Board of Registration in Social Work is required. - Based on assignment, travel may be required.
