St. Luke's Health System Pharmacist - Medication Therapy Management (MTM), Clinical in Boise, Idaho

Overview

St. Luke’s Health System in Boise is seeking a Clinical Pharmacist to join our Medication Therapy Management (MTM) Clinical team.

Position Summary

The MTM Clinical Pharmacist is responsible for providing personalized and patient-centered MTM services for the purpose of optimizing medication therapy to improve clinical, economic, and humanistic outcomes. The MTM Clinical Pharmacist participates in diverse services including comprehensive medication reviews, targeted interventions, adherence monitoring interventions, cost-effective interventions, transitions in care, and post-discharge medication reconciliations to achieve these outcomes.

Responsibilities

  • Coordinate and collaborate with interdisciplinary teams and St. Luke’s Integrated Health Technologies (IHT) to lead efforts that support the expansion of MTM services and post-discharge medication reconciliations

  • Conduct comprehensive medication reviews of patients' medication regimensAssess patients' health conditions, medical history, medication list and lifestyle to identify potential medication-related problems

  • Collaborate with patients and healthcare providers to ensure coordinated and integrated patient care and transitions of careevelop and implement personalized medication therapy plans to optimize effectiveness and minimize medication side effects

  • Provide patient education on proper medication use, potential medication side effects, and lifestyle modifications

  • Monitor patients' responses to medication therapy and make necessary recommendations to patients and providers for adjustments

  • Maintain accurate documentation of patient assessments, interventions, and communications

  • Participate in continuous professional development and quality improvement initiatives related to medication management and patient outcomes

  • Participate in continuous professional development to stay informed about new medications and healthcare trends

  • Develop protocols for application within the outpatient pharmacy setting in alignment with expansion of MTM clinical services

  • Serve as a role model and mentor to technicians, students, residents and other healthcare practitioners

  • Provide resources for cost-effectiveness and/or financial assistance as neededPerform other duties, responsibilities and clinical expansion opportunities as assigned

Qualifications

  • Degree from an accredited pharmacy program

  • Valid Idaho Pharmacist License

  • Must meet St. Luke’s credentialing requirements for MTM services within six (6) months of starting

Preferred qualifications

  • One (1) year experience in MTM or completion of formal MTM training course

  • Current Board Certification in Medication Therapy Management

  • Residency in MTM or Ambulatory Care Pharmacy (PGY-1 or PGY-2)
