Position Details

Full time, flexible schedule: benefit eligible

Working hours are on site

Position Summary: The Speech Language Pathologist evaluates and treats residents who demonstrate disorders of speech, language, cognitive communication, voice fluency and swallowing. Evaluation and treat will only be conducted after a written order is received from the physician.

Education:

Master of Science Degree in Speech Language Pathology.

Current NYS Licensure or proof of registration to complete the Clinical Fellowship year in Speech Pathology after which certification can be obtained.

ASHA certification or proof of ASHA certification.

Essential Job Duties and Responsibilities: The Speech Language Pathologist is responsible for the screening of residents of the faculty as follows. - New facility residents - within 24 hours of admission. Long Term residents - upon receipt of a significant change form and or referral from nursing.

The Speech Language Pathologist is also responsible for the completion of all necessary documentation, which includes but is not limited to the following items. Initial evaluation forms, weekly progress notes, monthly recertification forms, discharge summary and all other required forms as designated by the facility and Director.
