JOB REQUIREMENTS: Overview Beloit, Wisconsin Full-time, exact scheduling based on clinic needs This position provides direct service to patients in a Behavioral Health Program area. Responsibilities * Completes psychosocial assessments and treatment plans in cooperation with patient and multidisciplinary team. * Conducts a comprehensive assessment of the clinical needs of each patient prior to beginning individual, family or group therapy. * Works collaboratively with the patient to develop a treatment plan addressing patient\'s identified treatment goals. * Delivers behavioral health care in accordance with patient evaluation, treatment plan and within the scope of their license. * Works directly in treatment relationship with patients, individually or in groups, in consultation with Psychiatrist, NP. * Provides crisis assessment and intervention. * Consults and coordinates care of patient with physician, NP, therapists and community agencies. * Provides appropriate discharge planning. * Refers patient or patient\'s family to other community resources. * Participates in clinical collaboration and or psychiatry supervision to discuss cases and determine appropriate course of treatment. * Maintains pertinent patient data and documentation in medical record in accordance with state, TJC and Mercyhealth system guidelines. * Attends required meetings or staffing as indicated by Program Manager. * Participates in performance improvement activities. * Performs other duties as assigned. * Complies with all Mercy Health System, State and JCAHO policies and regulations.
