Great Lakes Bay Health Centers Behavioral Health Therapist in Bay City, Michigan

DEFINITION

Integrated (IBHS)-The Integrated Behavioral Health Consultant role addresses behavioral health needs and health behaviors in the medical setting.

Substance Use Disorder (SUD)-The Substance Use Disorder Behavioral Health Specialist provides counseling and support to individuals experiencing addiction. Assists the individual in their recovery process by using strategic interventions.  

Traditional-The Traditional Behavioral Health Specialist is responsible for providing diagnostic assessments, treatment plans, and evidenced-based therapeutic interventions to adults with various mental health diagnoses to assist them in developing meaningful behavioral changes.

Children/Adolescent- The Children/Adolescent Behavioral Health Specialist completes an assessment and ongoing outpatient behavioral health therapy to children and their families to address their mental health concerns. Coordinates and collaborates with child/adolescent psychiatry when needed. Collaboration with children's school and assists with making appropriate referrals to services when needed.

Mixed- A combination of IBHS, Traditional, and/or SUD Behavioral Health Specialist roles.

Maternal Infant Health Program (MIHP)-The MIHP Behavioral Health Specialist completes assessment and ongoing outpatient behavioral health therapy to and post-partum mothers within the Maternal Infant Health Program. Collaborates with case managers and medical providers to ensure patients mental and physical health are being address.

Accepts referrals from care team regarding patients' behavioral health needs. Responds in a timely manner and in a form of communication that is collaborative and reaches the care team effectively. Provides and documents referrals as needed to appropriate providers/resources both within and outside GLBHC.

Provides comprehensive assessment, diagnosis, consultation, and interventions for behavioral health, and/or substance use needs, and/or social determinates of health needs. When assessment indicates, recommends referral to a different specialty provider, other member of the care team, and/or consultation.  Provides counseling/behavioral health services to patients on an individual, couples, family, and/or group basis.

Assists the care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic conditions (mental and physical).

Consults and collaborates with patient and care team regarding the recommended behavioral health intervention(s). Through a consultative and collaborative relationship, communicates the behavioral health intervention(s) as a recommendation to the patient and care team.
