Amergis RN Care Manager - $44/HR in Barrington, Rhode Island

Amergis Healthcare Staffing is searching for a registered nurse care manager to work within health center in Barrington, RI.

-- Monday-Friday, Days, 8am-4:30pm

-- Local Pay Rate: $44/HOUR

  • Provide a wide variety of skilled office-basedand community-based nursing services in support of patient treatment plans and documentedorders by physicians, and/or advanced practice registered nurses, and/or physicianassistants within scope of license and experience

  • Provide ongoing health assessment (identifyhealth issues, behaviors, needs, barriers, etc) and all other assessments whichare appropriate for the nursing scope of practice. Ensure that all screening tests are up-to-datewith results recorded in the electronic medical record.

  • Triage walk-in patients as well as conducttelephone triage to determine patient need and disposition

  • Properly document all nursing visits in EBCAP’sElectronic Health Record (EHR), including, but not limited to: injectable medications, medicationreconciliation, medication education, administering of NARCAN/Naloxone, vitals,and toxicology screenings

  • Collaborate and regularly liaise withinternal/external providers, internal/external pharmacies, internal/externallabs, and community agencies, such hospitals, long-term care facilities, andhome health representatives based on consumer’s health and wellness needs

  • Coordinate appropriate coordination of carebased on complexity of presenting issues, and in conjunction with key provider instructions

  • Follow-up with phlebotomists/clinical laboratoriesto ensure that lab results are returned and reviewed in a timely manner

  • Build relationships with medical providerswithin the organization and in the community, which will provide the nurse witha network of physical health resources

  • Record and administer medications prescribed bythe team physician/psychiatrist; evaluate and chart medication effectiveness,complications, and side effects. Complete Prior Authorization procedures based on patient’s insurancerequirements as needed

  • Obtain verbal/telephone medication orders perCenter’s established practices and State licensing regulations

  • Request, follow-up, and obtain current and pastclinical records from primary care, and other external medical and/orpsychiatric records, in support of a holistic approach to clinical care

  • Develop and maintain disease or problem-focusedpatient plans of care

  • Participate in all daily or organizational teammeetings and clinical reviews, including, but not limited to: multi-disciplinary team meetings,multi-agency meetings, “No Wrong Door” clinical meetings, etc

  • Participate in the development, implementation,and utilization of an electronic medical record and electronic prescribing

  • Promote infection control practices within theCenter per established policies

  • Complete all necessary documentation incompliance with State and Regulatory Agency policies and procedures, and withpublished Center standards

  • Perform other duties as assigned by supervisor
