Department: Penn Care at Home Transitions Team

Shift: Full time, day shift, start time around 9:00. One weekend a month and rotating holidays.

About the Role:
As a Medical Assistant at Penn Medicine, you will play a crucial role in our healthcare team within a remote clinical call center environment. Your primary responsibility will be to assist in the delivery of healthcare management, ensuring a patient-focused approach and supporting the delivery of top-notch care. You will work under the direct supervision of a clinical manager, providing indirect patient care through patient interviewing, technical tasks, and communication with patients and their families.
Key Responsibilities:
Patient Care Accountabilities:
Provide excellent customer service to inbound callers by quickly identifying and assessing their needs, coordinating services, and resolving or directing concerns to the appropriate individual or department.
Assist Call Center Nurses in collecting patient data, including reviewing past medical history, vital signs, chief complaints, symptoms, concerns, and current medications for assessment by licensed staff.
Document patient information in the medical record, including updated vital signs, medications, test results, and patient concerns.
Maintain clinical and administrative knowledge base and designated skill set to meet established goals for daily call metrics and defined individual/team goals.
Understand insurance restrictions for durable medical equipment (DME) and supply ordering, lab work, referral needs, etc.
Assist in ordering or processing DME and supplies, and coordinate the scheduling of services (e.g., mobile phlebotomy) or referral needs on behalf of the clinical care team.
Ensure care coordination and complete communication (closing the loop and follow-up) between internal and external individuals in an appropriate and timely manner.
Employee Health Accountabilities:
Administer vaccines and read PPD results under the direction of licensed staff.
Manage specimen collection and testing, including performing phlebotomy and anterior nares nasal swab collections.
Perform laboratory and screening tests, record results in EMR, reconcile with logs where appropriate, document and report findings to providers, and perform all quality controls as per Point of Care Testing guidelines.
Prepare specimens for transport as necessary.
Maintain work area, treatment area, and testing areas in a clean, safe manner; restock supplies and equipment as needed.
Clerical Accountabilities:
Manage office telephone systems, including answering calls and returning messages.
Participate in responsive and proactive telephonic patient care, including care coordination with specialists, ancillary services, hospitals, labs, other home care agencies, and all other involved health services.
Obtain and scan documents as appropriate and required for delivery of care, including test results, faxes, reports, notes, referrals, etc.
Complete medical forms and other clinical requests for information as appropriate and required.
Prepare and send patient correspondence as required.
Assist clinical staff with the maintenance of patient education materials and supplies as needed.
Maintain confidentiality of all materials handled within the Network/Entity as well as the proper release of information.
Schedule patient appointments and maintain communication to confirm future appointments.
Document patients' medical information, including medical history and health insurance information.
Other/Regulatory:
Ensure compliance with all applicable federal, state, and local regulatory standards (e.g., TJC, DOH, FDA, HIPAA, HCFA, DPW, LCGME, SCGME, etc.).
Be flexible and readily adopt new processes and engage in practice operation changes.
Qualifications:
Education: High School Diploma/GED required. Completion of an accredited Medical Assistant program or equivalent experience required.
Experience: 7+ years of experience as an MA may be considered in lieu of program completion. At least 1 year of related experience preferred. MA Certification is a plus.
