Job Summary and Qualifications

The Stroke Coordinator is a Registered Nurse who provides support and assists in the delivery of quality patient care. The primary responsibility is continuum of stroke care for patients. The Stroke Coordinator will ensure continuity between clinical practice and standards of care to patients with diagnosis of stroke. This individual works collaboratively with physicians, nurse and case managers and other health care disciplines utilizing a team approach. This position will serve as a leader, role model and mentor to facilitate each professional providing care to stroke patients. This individual will report directly to the CNO as an in-house clinical consultant regarding excellence in clinical care.

  • Serves as liaison for the Magnet Program inclusive of oral communication and clarification submission of required documents

  • Conducts annual evaluation of each unit for evidence of the Forces of magnetism, presents analysis of unit strengths and weaknesses to Nursing Leadership.

  • Assumes responsibility for the completion and submission of the Magnet status application and re-designation.

  • Develops a timeline for data submission and meets all deadlines.

  • Oversees site visit related to Magnet status application.

  • Provides leadership for and participates on committees related to hospital Magnet status.

  • Assess needs to support ongoing Magnet status (i.e. increased staff certifications, publications, presentation, awards, etc.) and identifies and directs opportunities.

  • Coordinates staff education regarding the Magnet process

  • Performs comprehensive assessments of learning needs of staff with regards to magnet status.

  • Collaborates with the Director of Clinical Excellence to develop educational programming related to magnet status

  • Ensures organization awareness and understanding of Magnet status.

  • Provides consultation and support to staff with regard to pursuing publications, submission of abstracts, awards, and site visits.

  • Responsible for the development and coordination of the AHMC Stroke Program.

  • Represents the facility in community awareness programs, organizations concerned with cerebrovascular disease, and industry health screenings.

  • Responsible for coordinating the care of acute stroke patients across the care continuum.

  • Verifies baseline and functional measurements done by Nursing staff.

  • Assists the nurse in modifying the plan of care as needed based on baseline and functional measurements and patients progress toward expected outcomes.

  • Oversees implementation and adherence to established treatment protocols and clinical trial protocols.

  • Provides education to staff on appropriate protocols.

  • Prepares appropriate reports comparing AHMC to national data and assists on updating care protocols to reflect national changes.

  • Collaborated with Nursing leadership on development and promotional neurological nursing standards of care.

  • Develops standardized patient teaching materials for use by Nursing and Rehab staff.

  • Analyzes educational needs of stroke patients and carries out individualized teaching plan with patients and families.

  • In collaboration with multidisciplinary team, develops critical pathways for acute stroke patients.

  • Demonstrates knowledge of specific disease process, and treatments, intervention and response based on age specificity.

  • Collaborates with and communicates the Plan of Care to patient/family.

  • Is knowledgeable of the need and knows how to report suspected and identified medical/health care errors to their supervisors and/or through the facility occurrence reporting system as evidenced by verbalizing examples of reportable medical errors and appropriately submitting occurrence reports.

  • Is knowledgeable of the need to report actual and potential patient safety issues to their supervisor and/or through the facility occurrence reporting system as evidenced by verbalizing examples of reportable patient safety issues and appropriately submitting occurrence reports.

  • Demonstrates understanding of compliance with the six national patient goals as appropriate to their role.

  • Recognizes the right of patients to appropriate assessment and management of pain.

  • Screens all patients for pain and performance a more comprehensive assessment if pain is indicated and documents appropriately.

  • Performs pain assessment utilizing age appropriate measure and documents appropriately

  • Performs pain assessment of the special needs patient utilizing appropriate measure and documents appropriately.

  • Assesses and documents patient/family/SO learning needs, abilities, preference and readiness to learn based on age specifically and level of understanding.

  • Adhere to all provisions of EMTALA

  • Immediately responds to emergencies or acute changes in patient status.

  • Communicates patient care needs with physicians and plans for appropriate interventions

  • Delegates’ tasks to personnel based on current competency and patient care needs.

  • Demonstrates skill and knowledge base of all routine and specialized equipment used in the performance of duties.

  • Maintains working area and equipment in accordance with environmental and safety regulations

  • Sequesters defective equipment and sends appropriate work order

  • Anticipates the need for equipment and supplies

  • Assures all quality controls are performed as required.

  • Maintains confidentiality, security and integrity or patient and organizational data

  • Completes and submits annual demographic forms. Works with appropriate departments to obtain relevant data

  • Analyzes and summarizes reports, statistics, and other material to support report submissions.

  • Synthesizes information in a concise, logical manner and develops written and verbal reports as needed.

  • Demonstrates knowledge of and complies with infection control policies and applicable.

  • Immediately disposes of biomedical waste in designated container.

  • Communicates actual/potential infection control concerns to the Infection Control Coordinator in a timely manner.

  • Demonstrates good hand washing techniques

  • Assures personal protective equipment is available and appropriately used

  • Completes department-specific competencies annually

  • Provides evidence of annual continuing education and adheres to the Attendance Policy

What qualifications you will need:

  • RN License from State of Florida

  • Bachelors Degree

  • Current BCLS certification and ACLS
