Job Summary and Qualifications

The PPS Coordinator provides coordination for timely and accurate completion of the IRF Patient Assessment Instrument (IRF-PAI). This individual provides clinical support in the completion of data contained in the IRF-PAI to ensure accuracy in the scoring of functional status and reflection of the patient’s care needs. In addition, this position assists in the ongoing monitoring of identified benchmarks for the inpatient rehab population. The PPS Coordinator will have access to confidential patient information and maintains confidentiality of all information.

What you will do in this role:

  • You will assist and coordinate intake and pre-admission screening processes. You will document evidence of rehab admission criteria within established timeframe.

  • You will perform on-site and off-site clinical evaluations to assist Rehab Medical Director in establishing patient eligibility for admission.

  • You will provide information to patient/family/caregiver related to inpatient rehab and program features. You will communicate expectations and requirements for participation in the rehab program and conduct or coordinate tours of the rehab unit.

  • You will obtain information relating to patient resources and healthcare insurance benefits.

  • You will demonstrate clinical knowledge of rehab admission criteria to appropriately identify and qualify rehab patients.

What qualifications you will need:

  • 1-3 years' experience in an inpatient rehabilitation setting, preferred

  • License/certification; RN, PT, OT or ST

  • Other licensed clinicians (MSW) with 2 or more years direct exp in PPS role in inpatient rehab setting
