Cedar Valley Services Designated Coordinator in Austin, Minnesota
CVS has an opening for a Designated Coordinator at our Austin division. The Designated Coordinator is responsible for the development of individual service plans based on a thorough assessment of consumer interests and abilities; coordinating this plan with other staff and outside team members. Caseload is potentially comprised of Waiver and EE participants and may also include seniors and competitive placements. Will assure quality programming which meets DHS, DEED, and CARF requirements and other applicable regulations. Provides direction and support for direct support staff both in the facility and in the community.
