Job Summary and Qualifications

The HealthONE Crisis Assessment Team Clinician is responsible for providing services to patients who have complex psychosocial and discharge planning needs. In addition, the HCAT Clinician offers crisis intervention, community assistance, and, coordinates and facilitates the development of a Discharge Plan of Care for at-risk patient population with discharge needs. The HCAT Clinician also functions as a member of the Interdisciplinary Patient Care Team, who communicates and receives referrals from other patient care providers to help meet the needs of the patient and family for a safe discharge.

What qualifications you will need:

  • Requires provisional licensure in the State of Colorado as a , LPCC, LSW, LMSW, or equivalent licensure.

  • BCLS through the American Heart Association or American Red Cross required at time of hire and CPI also required within 30 days of hire.

  • Requires Masters Degree.

  • Minimum of 3-5 years of experience in psychosocial assessment, preferably within an emergency medical model.
