Provides leadership to Ambulatory Care Coordinators, Social Workers, and other clinical support staff. Reports to the Director of Ambulatory Care Nursing. Responsible for planning, directing, and organizing aspects of care facilitation within the ambulatory setting. This role will work with ambulatory leadership ensuring that the highest levels of patient care coordination and community services resources are provided and improved. This individual will be resourceful in nature by leading the ambulatory care coordination team, assisting providers and clinic leaders alike.
Position Requirements
Graduate from an accredited/approved school of nursing. MSN required within 3 years of hire. 3 years or more of experience in ambulatory clinic management and care coordination. Current American Heart Association or American Red Cross Basic Life Support (BLS). Current Registered Nursing License (Georgia) or qualifying multi-state compact license.
Preferred Requirements
Master's degree in Nursing. 5 years or more of experience in primary care management and coordinated community care efforts.
