PSYCHIATRIC DISCHARGE PLANNER (BS/BSW)

We are searching for a compassionate Discharge Planner to work in our Social Services department to ensure our patients have a solid plan for success when they leave our facility. We are looking for a degreed professionalt to coordinate discharge plans for individuals with mental health, psychiatric and addictive disease diagnoses.

Here is what we are looking for:

• Individuals with a bachelors degree in social work. Degrees in psychology, sociology or other related field of study may also be considered with relevant experience.

• Individuals with previous experience providing care in an inpatient or outpatient psychiatric or addictive disease setting

• Individuals with an interest in psychiatric, mental health and/or behavioral health

• Individuals with a passion for providing patient care in a team environment
