Craig's Doors Bilingual Case Manager in Amherst, Massachusetts

AMHERST Bilingual Case Manager Craig\'s Doors\' case managers provide comprehensive case management services to assist our guests with stabilization, achieving self-sufficiency, and moving from homelessness to permanent, stable housing. Services provided and created are done so with a humanistic approach, utilizing sociocultural competency. Responsibilities Be able to work in-person as this is not a remote position. Work with guests at our program locations in the Amherst and Hadley area. Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered Comprehensive evaluation and assessment of each individual\'s needs, strengths, limitations, barriers, and goals. Work with individuals to develop realistic service plans that identify issues and map a course towards their resolution. Provide long-term support as they work towards stabilization. Service plans may include assistance with housing, employment, income, education, health care, mental health, substance use disorders, disability support, domestic violence, legal and credit issues, or a combination of the above Assist with housing searches and applications for services such as veteran\'s benefits, social security, food stamps, cash and fuel assistance, etc. Make referrals to other agencies as needed Provide orientation to program guidelines and responsibilities Connect guests to community resources such as food pantries, schools, medical facilities, governmental offices, and public transportation If necessary, provide transportation to appointments or other necessary needs Maintain regular contact and scheduled meetings with guests on your caseload. Provide crisis intervention, either in-house or through outside agencies, and notify and provide documentation of such incidents to Craig\'s Doors management Maintain accurate, comprehensive, and timely case notes and files. Collect, enter, and maintain all necessary statistical information as well as any other documentation required by both Craig\'s Doors and outside agencies Develop and maintain relationships with area landlords and human service agencies in conjunction with the Case Management team and leadership Attend case-conference meetings with area officials and service providers Continually assess, refine, and develop service plans that will improve the lives of our guests while working with Site Managers and organization leadership Adhere to professional standards as outlined by protocols, rules and regulations Skills Must be fluent in Spanish, with a preference for Spanish being their first language Have knowledge of trauma-informed, person centered care and case management principles The ability to deliver effective communication and listening skills Have excellent organizational and time management skills Be fluent in problem solving skills and the ability to multi-task Be compassionate with teamwork skills Have a bachelor\'s degree in related field or 1-3 years relevant work experience Have reliable transportation and a valid drivers license Benefits: 401(k) 401(k) matching Dental insurance Employee assistance program Flexible schedule Health insurance Life insurance Mileage reimbursement Paid time off Professional development assistance Retirement plan Vision insurance Schedule: 8 hour shift Day shift Monday to Friday Work Location: In person
