RN Health Navigator

Full time

Key responsibilities include:

  • Serves as a care coordinator ensuring seamless and timely coordination of services throughout the continuum of breast health (breast imaging, surgery, medical oncology, radiation oncology, plastic surgery and genetics.

  • Identifies patient and family needs and helps facilitate access to services required to meet those needs.

  • Identifying and applying appropriate comparative data, investigating data quality problems and reporting issues.

  • Meets with patients and their families and helps the patient navigate the system.

  • Works as part of the nursing team in the Breast Center, performing all nursing duties for the smooth operation of the department. This nurse will assist with biopsy procedures and perform patient care as needed.

Responsibilities:

  • The RN is responsible for assessing the needs of each patient where assigned.

  • Planning the nursing care needed, including spiritual, emotional and physiological needs. Once planned, the RN is responsible for seeing that the plan of care is carried out through performance and or leadership of supervised personnel and that the plan of care is evaluated

  • The RN is directly accountable to the Nurse Manager for the quality of patient care delivered

  • The RN modifies and delivers care that is specific to the age, growth and development of patients according to the unit scope of services

  • When serving in the charge role, the RN is responsible for clinical and leadership functions, for assessing the needs of each patient and planning the nursing care needed to meet those needs

All individuals providing patient navigation services at the NAPBC-accredited program must demonstrate compliance with this standard by holding a qualifying healthcare certification that includes patient navigation training, or documented completion of competency-based training and education in patient navigation for patients with breast disease or breast cancer from a qualifying training program. Patient navigation training provided locally by the NAPBC-accredited program does not meet the measure of compliance for this standard.

Qualifying healthcare certifications must include patient navigation within their exam to meet the measure of compliance for this standard. Examples of such qualifying certifications with documented patient navigation training and education include, but are not limited to: • Oncology Certified Nurse (OCN®) • Certified Breast Care Nurse (CBCN®) • Oncology Nurse Navigator-Certified Generalist (ONNCG™) • Oncology Patient Navigator-Certified Generalist (OPNCG™) • Advanced Oncology Certified Nurse Practitioner (AOCNP®)

Qualifying training programs must include competency-based training and education in patient navigation for patients with breast disease or breast cancer to meet the measure of compliance for this standard. Examples of such qualifying training programs include, but are not limited to: • National Consortium of Breast Centers (NCBC): Certified Navigator Breast Advocate Program (CN-BA) • George Washington University School of Medicine and Health Sciences Education Program: Oncology Patient Navigator Training • Oncology Nursing Society: Equipping the Novice Oncology Nurse Navigator: An ONS Collaboration with AONN+ • American Cancer Society Leadership in Oncology Navigation (ACS LIONTM)

Pay range: $31.50-$45.70

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
