Universal Health Services RN - Care Transition Coordinator (Case Management) in AIKEN, South Carolina

Transition of Care (TOC) Coordinator - Full-time (40 hours)

Monday - Friday 8am - 5pm

The Transition of Care (TOC) Coordinator is a Registered Nurse, Case Manager who is a healthcare professional that is responsible for coordinating and managing the discharge process for patients in an acute hospital setting. Their primary role is to ensure a smooth transition from the hospital to home or the next level of care, optimizing resources for the patient's recovery and successful healthy living environment.

Key Responsibilities:

· Assess patient's condition and needs to develop an effective discharge plan

· Ensure smooth transitions between different levels of care, such as from the emergency room to the medical unit or from the hospital to home or a rehabilitation facility.

· Collaborate with physicians, nurses, and other members of the interdisciplinary team to implement the discharge plan.

· Coordinate and monitor the progress of the patient's care plan, working closely with the clinical team.

· Communicate and explain the discharge plan to patients and their families, addressing any concerns or questions they may have.

· Arrange necessary aftercare services such as home care, physical therapy, medical equipment, or follow-up appointments.

· Work closely with insurance providers to understand patient's coverage and benefits, and to ensure the necessary services are covered.

· Advocate for the patient's needs and preferences, ensuring their voice is heard in decisions about their care.

· Maintain detailed records of patient interactions and discharge plans, ensuring compliance with all relevant regulations and confidentiality requirements. Including data collection for hospital and state programs.

· Provide education and resources to patients and their families to promote health and prevent readmission.

    Qualifications

  • BSN preferred, g raduation from an accredited nursing program

  • Current licensure as a Registered Nurse

  • Minimum of 3 years in a hospital setting, preferably in a discharge planning or case management

  • Excellent communication and interpersonal skills

  • Experience in facilitating change through interdiscilpnary collaboration.

  • Ability to handle high-stress situations and make critical decisions quickly

  • Strong organizational skills and attention to detail

  • Recent working knowledge of Interqual criteria and Milliman Care Guidelines preferred.
